The 5 Stages of Stroke Recovery

Stroke recovery follows a predictable biological progression driven by neuroplasticity — the brain's capacity to rewire itself. Understanding each stage helps families set realistic expectations and helps therapists design the right interventions at the right time.

Stage 1: Flaccidity (Days 1–7)

Immediately after stroke, affected muscles become completely limp and unresponsive. This is called flaccid paralysis. The priority here is preventing secondary complications: joint contractures, pressure sores, and deep vein thrombosis.

Stage 2: Spasticity Emerges (Weeks 2–4)

As the nervous system begins to recover, muscle tone returns — but often abnormally high. Muscles become stiff and resistant to movement. Early stretching, positioning, and neuromuscular electrical stimulation are critical here.

Stage 3: Synergy Patterns (Weeks 4–12)

Movements become possible but are linked in primitive "synergy patterns" — for example, lifting the arm causes the elbow to bend automatically. Therapy works to break these patterns and create selective, independent joint control.

Stage 4: Selective Movement (Months 3–6)

Patients gain increasing voluntary control. Fine motor tasks return. Walking becomes more fluid. This is the prime window for intensive, high-repetition task-specific training.

Stage 5: Relative Recovery (Months 6+)

Some residual impairment may remain, but functional independence is achievable for most patients with continued rehabilitation. Neuroplasticity continues long-term.
