It arises from a combination of muscle and joint irritation, altered movement patterns, physical deconditioning, and pain sensitisation. Specific causes include disc herniation (10%), spinal stenosis, spondylolisthesis, and sacroiliac dysfunction. Psychosocial factors (stress, poor sleep, work dissatisfaction) are now recognised as strong predictors of chronification.
Surgery is rarely needed for lower back pain. Evidence shows that for most conditions (disc herniation, spinal stenosis, non-specific LBP), outcomes with physiotherapy are equivalent to surgical outcomes at 1–2 years, while surgery carries procedural risks. Surgery is considered for cauda equina syndrome (emergency), progressive neurological deficit, or failure of thorough conservative treatment after 6–12 months.
